Insurance & Billing Information

We participate with many HMO's PPO's and Care Systems, and we do our best to help you get the most from your medical insurance. A patient services representative will call you before your first appointment to collect your insurance information, but please bring your insurance cards to your first appointment so we can copy them.

Insurance Coverage:  It's important to carefully check your plan coverage and network listings before your first appointment to make sure your treatment at MVC is covered. If your policy requires a referral from your primary care physician, make sure you get that authorization or your insurance company may refuse to pay for your care. Then you would be responsible for payment of your medical costs.

You should also confirm that the hospital where your surgery is scheduled is part of your insurance company's network and that your policy covers the specific type of operation you will be having.

Co-payments:  If your insurance requires a co-payment, it must be made at the time of service. Please bring a means of payment (cash, check, MasterCard or Visa) to your appointment or a service charge may be added to your account.

Estimates:  If you would like an estimate of the cost of surgery, call 952-929-6994. and you will be directed to the appropriate administrative office. (Since coverage varies, the figure may not be exact.)

Filing Claims:  Our administrative office will file a claim with your insurance carrier to obtain the benefits you deserve, but you must take care of any portion of the costs listed as "patient responsibility."

Financing:  We offer financing options to cover your costs, and we accept cash, checks, MasterCard or Visa. In some cases, you may be asked to make a full or partial payment for surgery in advance. We will notify you if prepayment is required.

Medicare:  We participate in the Medicare program and our administrative office can submit your claim to Medicare as well as any supplemental insurance carriers.
